How do I know when I need a facelift?

Let’s face it; no one needs a facelift. The more important question is why do men and women reach a point where they are considering facial rejuvenation? It’s a conundrum. The years tick by, but in our hearts, we still feel youthful exuberance with just – perhaps – a touch more wisdom than we had twenty or thirty years ago. Truthfully, women seem to bear the pressure of aging more deeply than most men. And ageism can be felt in the workplace and dating. Many people in their 60’s and 70’s wish to or need to continue working and find themselves competing against a youthful workforce. Several have spoken about finding themselves “sidelined” or are asked when they might be retiring. The increased use of “Zoom” meetings and selfies has made people of all ages more self-critical, but for many who have begun to see the effects of time, gravity and sun damage, the self-criticism can build to a loss of confidence. That moment when you look in the mirror and the reflection doesn’t reflect who you feel inside is often the moment that patients pick up the phone to schedule a consultation. The art of aging gracefully does not necessarily require the foregone conclusion of wrinkles, jowls and sagging neck.

How long does a facelift usually last? Is it a permanent procedure?

Unlike threads, a deep plane facelift provides a long-lasting result. While each patient is unique, most patients feel they have turned back the clock 10-20 years. One must remember, of course, that you will, of course, continue to age. You just set the clock back on the aging process. 

Will my facelift scars be visible?

Dr. Claytor positions the scars within the fold of the ear, behind the ear and in the hairline so as to be as invisible as possible.  

There are a few options to choose from when it comes to facelifts. How will I know what type of facelift procedure is right for me?

That’s a great question. Some patients come in concerned only about their neck.  If they are young and have a thick or heavy neck, we can often address this with liposuction and a minimally invasive MyEllevate procedure. If the patient has loose or crepe skin (eg: turkey neck), they have excess skin, and this can only be addressed by removing and lifting the skin and addressing the underlying musculature of the neck. A neck lift can address the neck and the lower face, but it will not address the mid face. The deep plane facelift addresses the neck, lower face and midface, and provides a comprehensive rejuvenated result.

How long does it take to recover from a facelift?

You should give yourself two weeks to recover from a deep plane facelift. While the bruising is typically fairly minimal, you will experience some swelling, and you will want to make sure you stay well hydrated and rested while you are recovering.

Are there any non-surgical options that can achieve facelift-like results?

There is a multi-billion dollar industry that insists that facial rejuvenation can be achieved without surgery. There are lasers that are incredibly effective at improving skin tone and texture, and there are some treatments that can even help with skin tightening. Fillers (like Juvéderm, Restylane and Sculptra) and toxins (like Botox, Dysport and Daxxify) can help for a time. Patients come to me when they have tried these treatments and still have the same facial aging concerns they started with. The deep plane facelift is the holy grail of facial rejuvenation.

What’s one thing you wish people knew before receiving a tummy tuck?

When we talk with patients about their tummy tuck experience, the one thing we hear over and over is “I wish I had done this sooner!”  The confidence patients gain is evident; they love the way their clothes fit, how they see themselves reflected in the mirror – and nearly every patient comments on how they haven’t been able to wear a bikini in, well, forever, but they can’t wait to go shopping to choose one! A tummy tuck is an investment in oneself and can result in decades of self-confidence. 

How long does it last?

Great question. A tummy tuck will give you the sculpted, tight – or snatched, we say today – abdomen that so often eludes diet and exercise. The tummy tuck will tighten the rectus diastasis that plagues so many, particularly after childbirth and weight fluctuations. This muscle repair is permanent. That bulge? Gone.  Excess skin? Gone. Sculpted waist and flanks? Oh, yes. With that said, however, we continue to age. Skin will lose elasticity, but with a healthy diet and continued fitness, your results will last.

Are there different types?

Yes. Each body habitus and each patient’s desires and frustrations are unique. We listen to our patients goals and together, we come up with a plan to address their frustrations and achieve their particular goals.  

  • Standard Tummy Tuck: Addresses the rectus diastasis, tightening the separated muscles as well as tightening all the abdominal skin and muscles.  Dr. Claytor will liposuction excess adipose tissue in the abdomen and mons to achieve a sculpted result.
  • Modified Tummy Tuck: Tightens only the abdominal muscles or only the abdominal skin.
  • Extended Tummy Tuck: Tightens the abdomen as well as the flanks.
  • Mini Tummy Tuck: Tightens the lower abdominal wall skin only (below the belly button). Not all patients are candidates for this procedure.

What procedures can be combined with a tummy tuck?

Many female patients combine the tummy tuck with what we like to call “breast rejuvenation.” Breasts can lose volume, become asymmetric, or become droopy. These issues can be addressed with a mastopexy to lift the breast tissue back to its more natural position and/or breast augmentation to replace lost volume. Some patients are choosing fat grafting to replace lost volume. This entails liposuction of adipose tissue from one area and grafting the fat to the breast. For men, gynecomastia is sometimes added in order to reduce breast tissue. This can be done either with liposuction or as a surgical reduction. 

How long can recovery take?

I have a cheeky way of telling my patients that in a month they can go skydiving. That’s my way of saying they can do anything they want – climb a mountain, jump out of an airplane, go back to full workouts at the gym – or book that long awaited trip to the tropics so they can show off that new bikini.  

The first few days are the hardest, of course. Because I never use drains when I perform tummy tucks, I have patients moving about more quickly. I want patients up and moving on day one, showering the day after surgery.  By week two, they will be at half speed, and by week four? Well, anything goes!

You should be at an ideal weight you can easily maintain before you choose to undergo tummy tuck surgery. And you should be committed to maintaining a healthy lifestyle in order to retain your results. For some, liposuction can be an option, but as far as non-surgical, there is not an option that will enable you to achieve a tighter, sculpted abdomen.

Neuromodulators (i.e. Botox)

Neuromodulators are injected into facial muscles to relax them, reducing expression-based lines like those in the forehead or at the corners of the eyes. While you have likely heard of Botox, other neuromodulators like Daxxify™, Dysport®, and Xeomin® work the same way.

Men have stronger facial muscles and different aesthetic goals compared to women. Consequently, the application of neuromodulators in men requires a tailored approach. Men often seek a more subtle effect that maintains a certain level of ruggedness or masculinity, rather than aiming for a completely smooth, wrinkle-free appearance (although it’s not uncommon for preferences to differ among men requesting this treatment). 

Why is men’s Botox more expensive?

Men typically have larger, stronger facial muscles, requiring more units of neuromodulator to achieve the desired effect. Neuromodulator pricing is calculated in small units, making men’s neuromodulator treatments generally more expensive than women’s.

Men’s Botox & dermal filler differ from women’s—men often need more Botox, but less filler, with somewhat different injection patterns.

What is the best age to start Botox for men?

There’s no one-size-fits-all answer to the best age for starting neuromodulators. It varies depending on individual factors like skin type, lifestyle, and genetics. However, many men begin to consider neuromodulators in their late 30s to early 40s when dynamic wrinkles in the upper face start becoming more pronounced. Starting neuromodulator treatments at a younger age can serve as a preventative measure, potentially reducing the severity of wrinkles in the future.

Dermal filler

Dermal fillers are gel-like, biocompatible substances that are injected into the skin to smooth wrinkles and deep lines or enhance certain features, like the jawline and chin. The goal of dermal fillers is to recreate a more youthful appearance, and because a youthful masculine face shape is different from a youthful feminine face shape, filler must be injected with a gender-specific approach.

Anatomy

A youthful look in men typically involves a more pronounced brow ridge, a stronger jawline, and thicker skin compared to women. If you want to look more masculine and young, filler should be considered in the jawline and brow areas, in addition to touching up any deep lines or wrinkles, as desired.

Anatomical placement of filler will also affect the chosen product used: there are thicker, more volume-building fillers are ideal for emphasizing the jawline and other bony areas, while thinner, more flexible formulas may be ideal for lips and cheeks. Aesthetic goals

The aesthetic objectives for men and women receiving dermal fillers often vary. Women usually ask to emphasize features like full lips and high cheekbones. The study found postsurgical scar appearances are significantly improved after a series of microneedling treatments in early healing phases.

Dr. Claytor’s paper showcasing the novel approach to treating post-surgical scars with microneedling treatments stood out by a distinct margin, proving its merit as a leading paper in its category.

“Percutaneous collagen induction has achieved noteworthy improvement in acne scars for years,” says Dr. Claytor. “My goal for this study was to determine if microneedling treatments could produce similar improvement for surgical scars and develop new treatment protocols. I believe we have been very successful.”

The ground-breaking study consisted of twenty-five patients who underwent surgery and received a series of three microneedling treatments to their scars with the goal of increasing collagen production. The patients’ scar appearance was evaluated after each treatment and at a final 2-month follow-up. The data showed that treating surgical scars 6-7 weeks postoperatively showed superior improvement compared to scars that were treated 13-16 weeks postoperatively; no adverse effects were observed. Patient’s age, skin tone, or scar location did not appear to affect the outcome of their results.
