New study finds CO2 laser, microneedling, and nanofat can be combined for unprecedented results and less downtime

PHILADELPHIA, May 3, 2023 /PRNewswire/ — A March 2023 study in Aesthetic Surgery Journal found that combining CO2 laser and microneedling anti-aging treatment with nanofat application, or fat taken from one area of the body and applied to the skin, leads to a speedier recovery and less pain. The study authors have dubbed this novel approach “LaMiNa” (Laser, Microneedling, and Nanofat).

“We are thrilled with the results of this study,” said Dr. Brannon Claytor, the study’s lead author and a nationally-renowned expert in facial rejuvenation. “LaMiNa is a game-changer for patients in need of an efficient, effective non-surgical anti-aging option, as treatment is quick and easy for the patient, yet still offers impressive results when it comes to reducing the appearance of perioral wrinkles and other signs of aging.”

Tissue samples, in addition to patients’ before and after photos, showed noteworthy results: when laser and microneedling were combined with nanofat, skin cells showed less inflammation 4 days post-procedure, indicating quicker healing. Additionally, there was evidence of increased cell turnover—the process that leads to a more youthful skin appearance.

Both laser skin resurfacing and microneedling are well-established mainstays for improving skin texture, softening the appearance of wrinkles, and reducing the visibility of age spots. They do so by selectively damaging the skin to promote a healing response, in which the body supplies the skin with new healthy cells and increased collagen and elastin. But significant skin resurfacing treatments typically come with side effects: pain, swelling, redness, discoloration, or peeling are common.

Common scar care methods after plastic surgery

To speed along the process of scar “maturity,” plastic surgeons routinely advise their patients to use scar strips, scar gel, and even scar massage. But in recent years, collagen remodeling treatments like laser treatments and microneedling have also emerged as effective scar-minimizing techniques.

Microneedling, also known as “collagen induction therapy,” is an innovative and minimally-invasive skin treatment typically used on the face to treat acne scars, fine lines and wrinkles, hyperpigmentation, dull skin tone, and enlarged pores. Very fine, hair-like needles are passed over the skin surface to prompt the body’s healing mechanism, promoting fresh collagen and elastin in treated areas for better plumpness, smoothness, coloration, and elasticity. Microneedling has been proven to be a sound treatment for frustrating chronic acne scars, and it is also commonly used for treating stretch marks, which are a form of scar.

While it is impossible to make a facelift or breast lift fully scarless, minimizing post-surgical scars with in-office treatments like microneedling is well-proven and can be a routine part of your cosmetic surgery treatment plan.

While laser skin resurfacing has been a more common option for minimizing post-surgical scars, board certified Philadelphia plastic surgeon Dr. Brannon Claytor saw the effectiveness of microneedling for treating other skin irregularities and its promise for surgical scars. He thus spent the past several years conducting a study into its effects on our Philadelphia plastic surgery patients’ scars. In the study, conducted at our practice here in Bryn Mawr, 25 patients returned to Dr. Claytor for 3 microneedling sessions at regular intervals after surgery.

At just four months post-op, patients treated with microneedling saw significant scar improvement based on 3 different measures, including both doctor and patient ratings. Dr. Claytor’s study also found that those who started microneedling earlier—just 6 weeks after surgery, once the initial healing process was complete—saw even better results at the four-month mark. Previously, most surgeons believed that it was necessary to wait a full year after surgery before using microneedling to improve surgical scars’ appearance.

The research was published in the peer-reviewed journal Plastic and Reconstructive Surgery. Dr. Claytor is happy to offer this option in our office to his plastic surgery patients after most procedures for the face, breast, and body.

Facelift options in the Main Line

A deep plane facelift is ideal for older patients who have moderate to severe symptoms in the mid-face and neck, such as drooping cheeks, visible jowling, excess skin and fat below the chin, and sagging skin in the neck. A deep plane facelift can also address sunken cheeks and a hollow eye appearance.

A SMAS plication facelift benefits patients who have mild to moderate symptoms in the mid-face and neck and desire a more defined jawline, volumized cheek area, and tighter neck. This is achieved using layers of sutures in the SMAS that lift the skin laterally towards the hairline; any excess skin and fat are also removed. The underlying ligaments that are tightened in a deep plane facelift are not disturbed.

A full facelift is the traditional skin and fat excision procedure for patients who wish to achieve a considerable lift in the mid face and neck region.

A short scar facelift or “ponytail facelift” technique is excellent for younger patients and professionals who have mild symptoms in the lower mid-face and jawline.

A mini facelift is a great option for patients who find they need more than injectable dermal fillers to maintain a youthful appearance, but prefer minimal downtime.

Is deep plane facelift recovery different from SMAS facelift recovery?

Yes, slightly—because a deep plane facelift allows the skin and SMAS to remain attached, this technique causes less trauma so recovery time can be shorter. Facial compression wraps and wound care instructions will be similar between both facelift techniques. Most patients feel comfortable going out in public after about two weeks.

Yes; a deep plane facelift typically costs more as it requires additional skill and precision compared to other face lifting techniques. The technique goes beyond the SMAS to tighten the ligaments that support the muscle and skin and, during this process, the facial nerves that run along the ligaments must remain intact to avoid nerve damage. This means it is very important to choose a surgeon with proven deep plane facelift expertise, versus choosing a provider based on pricing alone.

Does a deep plane facelift leave more scars?

No; because the face is lifted at a deeper level with minimal tension placed on the skin, healing incisions will not be under stress and, once healed, any scarring should be nearly invisible. Additionally, deep plane facelift incisions are strategically placed around the contours of the ears and within the hairline to hide them from view.

How long do deep plane facelifts last?

There are a number of practical things to do in the weeks before as well as the day of your bridalwear appointment. Here are some key points:

Mother of the Bride Looking Radiant and Beautiful on the Big Wedding Day

As the mother of the bride or groom, you’re front-row and often center-stage for this exciting event that brings together friends and family from near and far. If we’re being honest, it’s your day, too! So as you prepare to celebrate your son or daughter’s wedding, it’s a good idea to schedule time to take care of you.

In addition to manis and pedis, many women take this opportunity to finally get the cosmetic procedure that’s been on their mind. Others undergo a few maintenance treatments to take care of skin spots, fine lines and wrinkles, or to enhance their naturally beautiful features. Below, we describe the top non-surgical and surgical procedures our Main Line and Philadelphia patients choose to prepare for their son or daughter’s wedding.

Top non-surgical treatments for mothers-of-the-bride

IPL facial

An intense pulse light (IPL) facial evens skin tone, gently tightens the skin, and minimizes the appearance of sun damage and pigmentation, including brown spots, freckles, sun spots, broken blood vessels around the nose, and rosacea.

Timeline: At least 2 months before the wedding; a series of 4+ treatments, spaced about 1 month apart, may be recommended for optimal results.

Many facial treatments achieve best results with a series of 4+ treatments, so aim to visit your plastic surgeon a few months in advance for an optimal transformation.

Laser skin resurfacing

Our fractional laser skin resurfacing treatment is a powerful anti-aging treatment, helping to firm the skin and improve its overall appearance with less downtime than previous generations of laser treatments. You’ll see a gradual improvement in tone, texture, fine lines, and tightness of your skin.

Timeline: At least 3 months before the wedding; a series of treatments (usually between 2 and 5), spaced 3-6 weeks apart, is recommended for optimal results.

Botox

Neuromodulator injections with Botox®, Dysport®, and Xeomin® are a wonderful way to smooth the skin in the upper face. All of these “dynamic” wrinkles are caused by muscle contractions, and neuromodulators work by relaxing those muscles to smooth the skin. Botox can even diminish platysmal bands—the vertical lines in the neck.

Timeline: At least 2 weeks before the wedding; results last 3-4 months.

Dermal fillers

We love dermal fillers for their versatile ability to provide precise enhancements, smooth wrinkles in the lower face, and replace lost volume in the face and hands. Fillers also provide longer-term collagen enhancements, even after they dissolve.

At Claytor Noone Plastic Surgery, we carry a range of product formulas that vary in active ingredients, how long they last, and which areas they are best suited for: Sculptra®, RHA® Fillers, Juvéderm®, Restylane®, and Bellafill®.

Timeline: At least 2 weeks before the wedding; results last 6 months or longer, depending on the product.

Top surgical procedures for mothers of the bride

Facelift and/or neck lift

A facelift or a neck lift turns back the hands of time by repositioning deep tissues and removing excess skin. A facelift (rhytidectomy) addresses sagging cheeks, jowls, and lifts and contours the neck; a neck lift focuses specifically below the chin.

Timeline: At least 3 months before the wedding; results continue to improve for a calendar year.

Lip lift

Over time, the skin above the upper lip loses collagen and lengthens, making lips appear smaller and out of proportion with our features. To achieve long-lasting fuller lips and reverse signs of aging in this area, a lip lift offers a very natural-looking solution. Instead of adding volume, a lip lift allows more of your natural lips to show, enhances your lips’ shape, and allows more of your teeth to show (as needed).

Timeline: At least 2 months before the wedding.

Minimally-invasive neck lift

The minimally-invasive neck lift with MyEllevate requires only a tiny, imperceptible incision for a lift that supports the underlying structures that appear to “fall” with age. With no lengthy incisions, no scars, and no general anesthesia, MyEllevate restores a well-defined, attractive jawline and chin. It is performed in about 2 hours in our office, and requires less than a week of recovery time. (You’ll need to avoid exercising or vigorous activities, however, for a few more weeks.)

Timeline: At least 3 weeks before the wedding.

Planning ahead for surgical procedures is important, since results may improve up to a year after surgery, and surgical scars continue to diminish over time.

Arm lift (brachioplasty)

An arm lift tones and tightens sagging skin in the upper arms (from shoulder to elbow) so that you can comfortably bare them at your son or daughter’s summer wedding. Through an incision along the inside of the upper arm, Dr. Claytor tightens muscles in the area and removes excess skin and unwanted fat pockets for a slimmer, tighter contour that looks elegant in sleeveless clothes.

Timeline: At least 3 months before the wedding; final shaping results are apparent and incisions will be fully healed after about 4-8 weeks. That said, arm lift scars continue to fade and improve over time—so if baring your upper arms at the event is a priority, you may wish to plan your procedure closer to a year in advance.

Eyelid lift (blepharoplasty)

Puffy, sagging, or tired-looking eyes detract from your overall appearance, and often make us look angry or tired even when we’re at our most joyful and alert. An eyelid lift removes excess tissue along hooded upper eyelids and tightens as needed in the lower eyelids—in effect, transforming your overall facial appearance.

Timeline: At least 2 months before the wedding; final results continue to improve subtly for up to several months.

Breast augmentation

Breast implants can improve breast size and shape to restore a balanced, proportional figure. A breast augmentation can be safely combined with a breast lift (the combination is called an augmentation mastopexy) to also raise the breasts higher on the chest, remove excess skin, and restore a natural-looking areola.

Timeline: At least 1 month before the wedding.

Tummy tuck

A tummy tuck, or abdominoplasty, transforms your figure by restoring a firm, smooth abdomen. Abdominal muscles are tightened (and any separation is corrected), and excess skin is re-draped for a naturally slim abdominal appearance.

Dr. Claytor is renowned for his drainless tummy tuck technique, a specialized procedure that is more than just a tummy tuck without a drain. A drainless tummy tuck involves leaving a thin layer of scarpa’s fascia (connective tissue) intact to allow the body’s natural lymphatic drainage system to eliminate fluids after surgery. This makes post-surgical drains unnecessary, while also improving the blood supply to the area and reducing the amount of traumatic tissue disruption.

Timeline: At least 1 month before the wedding.
